Apunipima provides comprehensive primary health care to 11 Cape
York communities via its Healthy Lifestyles and Family Health teams
Apunipima’s community driven, family centred comprehensive primary health care includes population health approaches. Our Population Health team works with communities and sub-groups across the lifecourse to improve health outcomes by addressing the various social and cultural determinants of health. We focus on a strengths-based approach to service delivery and practice, and incorporate Indigenous knowledges in planning, delivery, evaluation of services and programs.
Our Population Health Unit includes three main teams, Nutrition, Health Promotion & Integrated Team Care (care coordination for chronic conditions)
Current projects within these teams include: Tackling Indigenous Smoking, Remote Food Security – Research and Gather and Grow programs, My Health For Life, a group-based behaviour change program. Through these and our core work, we provide regular community focused programs providing health education, advocacy and supportive environments across a broad range of health topics, in response to community needs across Cape York.
Our team of qualified health professionals, many with family community links, come from diverse expertise backgrounds facilitating strong commitment and a solid program reputation tailoring evidenced-based approaches to meet community needs.
